Ivonne: Bueno, el Cirujano Oculoplástico José Raúl Montes nos acompaña para hablarnos del rejuvenecimiento del rostro inferior que delata la edad. Buenos días. Bienvenido.
Dr. JRM: Buenos días Ivonne y buenos días a todos los televidentes.
Ivonne: Bueno, ya lo dijimos, el rostro inferior delata la edad, pero antes vamos a hablar de algo importante. Usted nos abandonó por un tiempo. Estaba primero en Panamá y después en Japón. ¿Por qué está viajando tanto¿
Dr. JRM: Estaba trabajando Ivonne. Fui invitado a dos congresos importantes. Ya cumplí con el de Panamá hace una semana y en dos semanas ya estoy en Japón, así cubriendo ambos lados del mundo, representando a Puerto Rico.
Ivonne: Okay. ¿Como orador invitado?
Dr. JRM: Como orador invitado y hablando de muchas de las cosas que vamos a cubrir hoy.
La parte inferior del rostro generalmente se había pensado que las opciones que teníamos eran siempre de cirugía y los pacientes hoy día se resisten a procedimientos que le tomen más tiempo de recuperación, que sean más invasivos. Pues ellos mismos, la industria, a los médicos, nos han pedido buscar otras alternativas. Hoy tenemos alternativas para el rostro inferior que se conoce obviamente como la parte inferior que está debajo de la nariz comprende la boca, la mandíbula, el área debajo de la mandíbula o submental y en el cuello.
Tenemos alternativas, como vemos en este video, donde estamos inyectando sustancias de relleno en esta paciente. ¿Qué hacen estas sustancias de relleno, sobre todo en la mujer? Van a definir la mandíbula. Como vemos aquí, se pueden hacer en dos direcciones para crear definición, definición en la mandíbula y para crear soporte de la piel.
Ivonne: O sea, esta definición se pierde con la edad.
Dr. JRM: Eso es así. Aquí vemos precisamente un área donde no estamos inyectando relleno. Estamos disolviendo el exceso de grasa, así que estamos haciendo una combinación de tratamiento donde en áreas especificas donde se acumula la grasa. La disolvemos con ácido desoxicólico o Kybella, que se usa para la papada y sustancias de relleno que nos definen. Aquí vemos el resultado de esta paciente en la parte inferior de la cara, en una combinación de tratamiento, ácido hialurónico o Voluma y Kybella para reducir la grasa.
Ivonne: Bueno, también tenemos tratamientos para hombres, pero eso lo vamos a cubrir para la próxima ocasión porque se nos ha acabado el tiempo, pero en su próxima visita nos tiene que presentar algo para el público masculino que también quiere tratarse y mantenerse al día en lo que respecta a tratamientos.
Dr. JRM: Así será.

Dr. JRM: Good morning Ivonne and good morning to all viewers.
Ivonne: We already said that the lower face reveals your age, but before we go into that, let’s talk about something important. You left us for a while. You first went to Panama and then Japan. Why are you traveling so much?
JRM: Ivonne, I was working. I was invited to two important congresses. I just finished my trip to Panama a week ago and in two weeks I will be representing Puerto Rico in Japan, covering both sides of the world.
Ivonne: Okay. As a guest speaker?
Dr. JRM: Yes, as a guest speaker and discussing many things that we will be covering today.
There was a general idea that that the only choices we had always involved surgery, and nowadays patients resist procedures that are more invasive and involve more recovery time. The industry approached physicians and asked to find other alternatives. Today, we have alternatives to treat the lower face area which is obviously beneath the nose including mouth, jaw, the area below the jaw or submental, and the neck.
As we can see in this video, we now have alternatives wherein we inject patients, such as this one, with fillers. What do these fillers do, especially in women? They will define the jaw. As we see here, it can be done in two directions to create definition, definition in the jaw area and to create skin support.
Ivonne: So, this definition is lost with age.
Dr. JRM: That is correct. We can observe here an area where filler is not being injected. We are dissolving excess fat, with a combination treatment in specific areas where fat accumulates. We dissolve it with deoxycholic acid or Kybella, which is used for double chin, and fillers for definition. We can observe the results on this patient at her lower face, wherein we used a combination treatment of hyaluronic or Voluma and Kybella to reduce fat acid.
Ivonne: Well, we know there are also treatments for men, but we will have to cover on a next occasion, because we have run out of time, However, on your next visit we look forward to seeing your presentation on treatments for the male audience, who also seeks and wants to keep up with the latest treatments.
