Ivonne: Bueno, nuestro Cirujano Oculoplástico, Doctor José Raúl Montes, nos presenta un tratamiento para el rejuvenecimiento de los labios. Bienvenido doctor.
Dr. JRM: Muchas gracias.
Ivonne: Tenemos aquí a la paciente.
Dr. JRM: Vianca y a Dayna, mi asistente.
Ivonne: ¿Cuándo una persona necesita tratar sus labios? Porque no solamente se trata de rejuvenecer, porque ella es joven.
Dr. JRM: Hay dos grupos de edad (Ivonne). El grupo de edad de personas joven, como Vianca, que tienen el labio con volumen adecuado y forma adecuada. En estos pacientes se trata de una manera bien conservadora y simplemente el procedimiento se hace buscando resaltar esa estructura de la cara que son los labios y darle balance al resto de la cara para que los labios se resalten. En el paciente de más edad queremos reconstruir esa área donde se pierde volumen, los labios se afinan, se meten hacia adentro, hay líneas alrededor que las mujeres se quejan de que el lipstick o el lápiz labial se le va por esas línea y el labio se alarga. En esos pacientes el procedimiento se hace de otra manera, para reconstruir.
Ivonne: Vamos a ver cómo es el procedimiento, porque lo vamos a presenciar aquí en directo; se va a llevar a cabo el procedimiento para que nuestras televidentes tengan el privilegio de ver cómo es que se lleva a cabo. ¿Qué tipo de producto, doctor?
Dr. JRM: Generalmente, se utiliza ácido hialurónico; es el producto ideal, Ácido hialurónico, ¿por qué? Porque es un producto que se integra muy bien al labio; deben ser productos que no sean permanentes. Los malos ejemplos de procedimientos cosméticos en la calle son de pacientes que sea han hecho los labios desgraciadamente porque se han hecho el producto que no es el adecuado.
Aquí estamos usando ácido hialurónico, que se conoce como Volbella® y es un producto muy suave, que van a ver que se integra al labio; van a ver que yo doy un golpecito y esto es simplemente para distraer al paciente, para que no experimente ningún tipo de molestia o dolor. Ustedes van a ver en cámara hoy que yo hago la mitad de la boca para que vean la diferencia entre un lado y otro; después fuera de cámara terminamos el procedimiento.
Le voy a pedir a mi paciente que abra levemente; voy a moverme al labio inferior. Una de las cosas que siempre le digo al paciente es que yo voy a resaltar el cuerpo del labio, voy a evitar inyectar en el borde para que el tratamiento se vea natural y no se sepa que el paciente se ha sometido a ningún procedimiento.
Ivonne: Doctor, ¿existe resistencia muchas veces entre las mujeres a someterse a este tipo de tratamiento por ejemplos que vemos a diario de labios rellenos que lucen extremadamente artificiales? ¿Por qué?
Dr. JRM: En el pasado (Ivonne), utilizaban sustancias de relleno en los labios que no eran las adecuadas. Por ejemplo, se usaba silicón, que eso es un error grande. ¿Por qué? Porque son sustancias que son permanentes y en el labio deben utilizarse productos que se han hecho específicamente para esta zona, ácido hialurónico con la concentración adecuada.
Tenemos hoy día, de todas las casa farmacéuticas, el FDA [Food and Drug Administration] ha aprobado productos que son específicamente para el labio, por ejemplo, Belotero®, Restylane® Silk, Restylane Refyne™, Vollure™ y Volbella™, que es el que estamos utilizando hoy aquí.
Ivonne: ¿Alguna contraindicación? ¿Cuándo se va a ver la diferencia? Primero, ¿cuándo ella va a notar una diferencia en su labio, de inmediato?
Dr. JRM: Inmediatamente. Ahora mismo la van a notar y la van a ver en cámara. Por ejemplo, ustedes van a ver. No sé si tenemos en el acercamiento suficiente de cámara. Van a ver la mitad de su labio hecho y la otra mitad sin hacer. Eso es lo que estamos procurando.
Ivonne: ¿Y es un cambio sutil?
Dr. JRM: Bien sutil. Solamente el paciente mismo se debe dar cuenta; ni siquiera su pareja se va a dar cuenta.
Ivonne: Déjame hablar con ella. ¿Qué se siente, ahora mismo, inca un poquito? ¿Qué se siente?
Vianca: Realmente, nada. Puedo decir que bien leve la molestia al momento de la inyección, pero no hay dolor.
Ivonne: ¿No hay dolor?
Vianca: No.
Ivonne: ¿Alguna contraindicación, doctor?
Dr. JRM: En el momento de inyectar, el paciente no debe tener por ejemplo con ninguna inflamación o infección en el labio. Sabemos que por ejemplo los pacientes que puedan padecer de herpes o esas ulceritas en los labios, ese no es el momento de inyectarse. ¿Por qué? Porque hay una inflamación o una infección.
Ivonne: Buenos, pues ya podemos ver la diferencia entre un labio que está relleno y el otro lado del labio que no lo está. Le hemos brindado la oportunidad de que nuestros televidentes vean el tratamiento en vivo. ¿Toda mujer puede hacerse este tipo de tratamiento?
Dr. JRM: No solamente toda mujer, también los hombres se someten a tratamientos de los labios. Acuérdense que los labios son estructuras, que por tener mucho movimiento con el paso del tiempo se va perdiendo el volumen, se van entrando hacia adentro y le da edad a la persona. Así es que, para restablecer esto se utiliza este procedimiento. Se puede hacer de dos maneras (Ivonne). Hoy lo estuve haciendo con aguja, aquí como lo vieron en estudio; es puede usar con cánula, que simplemente es una técnica donde no se usa aguja y el paciente ni siquiera experimenta sangrado cuando se hace el procedimiento.

Dr. JRM: Thank you very much.
Ivonne: We have the patient here.
Dr. JRM: Vianca and Dayna, my assistant.
Ivonne: When does a person need to treat the lips? Since it is not just about rejuvenating, because she is young.
Dr. JRM: There are two age groups (Ivonne). The first group is young people, such as Vianca, whose lips have adequate volume and proper form. For these patients, the approach is very conservative. The procedure is simply done to enhance the face’s structure, to create balance among the lips and the rest of the face, so the lips are enhanced. In the second group of older patients, we want to restructure the area where volume is lost, the lips become thinner, they hide inward, there are lines surrounding them for which women complain about the lipstick getting in and the lip elongates. In those patients, in order to reconstruct, the procedure is done in another way.
Ivonne: Let’s see how the procedure is done, because we are going to see it live, so that our viewers have the privilege of seeing how it is performed. Doctor, what kind of product do you use?
Dr. JRM: Mostly, hyaluronic acid. Why? Because it is the ideal product, which integrates very well into the lip; they must be products that are not permanent. Bad cosmetic procedures cases occur when patients who have undergone a treatment for their lips have unfortunately used a product that is not suitable for the area.
We are using hyaluronic acid here, known as Volbella®, and it is a very soft product, which you will see as it is integrated into the lip. You can see that I give the patient a little tap simply to distract her, so that she does not experience any kind of discomfort or pain. On camera today, you are going to see me do half of the mouth, so that you can notice the difference between one side and the other, then off camera I will finish the procedure.
I am going to ask my patient to open slightly. I am moving to the lower lip. One of the things that I always tell the patient is that I am going to enhance the lip’s body, and avoid injecting on the edge, so that the treatment looks natural. The purpose is for no one, other than the patient, to know that the patient has undergone any procedure.
Ivonne: Doctor, too frequently, there seems to be resistance among women to undergo this type of treatment because of the cases we see daily where lips that have been filled look extremely artificial. Why?
Dr. JRM: Ivonne, in the past they used filling substances on the lips that were not adequate. For example, silicone was used, which is a big mistake. Why? Because these are substances that are permanent, and the only products that should be used are those that have been specifically made for this area, such as hyaluronic acid with the right concentration.
Nowadays, all the pharmaceutical companies have [Food and Drug Administration] FDA approved products that are specifically for the lip, for example, Belotero®, Restylane® Silk, Restylane Refyne™, Vollure™ and Volbella™, which is the one that we are using here today.
Ivonne: Any contraindications? When will it be noticeable? When is she going to see a difference in her lip, right away?
Dr. JRM: Immediately. Right now, everyone will notice it, if the camera can do a close up. You can notice half of her lip treated and the other half untreated, which is our approach so the audience can appreciate the difference.
Ivonne: Is it a subtle change?
Dr. JRM: Yes, very subtle. Only the patient should know; not even her partner is going to notice she got a treatment done.
Ivonne: Let me talk to her. How does it feel right now?
Vianca: Actually, nothing. There was a slight discomfort at the time of the injection, but there is no pain.
Ivonne: There is no pain?
Vianca: No.
Ivonne: Any contraindications, doctor?
Dr. JRM: When injecting, the patient should not have for example any inflammation or infection in the lip area. Obviously, patients who may suffer from herpes or those lip ulcers should postpone the treatment. Why? Because it cannot be done when there is an inflammation or infection.
Ivonne: Well, we can already see the difference between the side of the lip that is filled and the other one that is not. We have given our viewers the opportunity to see the treatment live. Can every woman get this type of treatment?
Dr. JRM: Not only every woman, but also men undergo lip treatments. Remember that the lips are structures, which have a lot of movement so throughout time the volume is lost; they tend to hide inward and age the person. In order to restore this, this procedure is used. It can be done in two ways (Ivonne). In the studio today I was doing it with a needle; it can also be done with a cannula, which is simply a technique without a needle, and the patient does not even experience bleeding when the procedure is done.
