Telemundo Telenoticias – 13 de septiembre de 2017 – Kybella® Tratamiento en Vivo
Charito: Ya estamos listos con el doctor José Raúl Montes, Cirujano Oculoplástico. Veremos la aplicación de Kybella en la papada, así que tiene en la mano derecha la aguja y en la izquierda va a coger la papada para empezar a aplicar. Esto fue un procedimiento que reseñamos tan pronto llegó a porque que fuiste el primero.
Dr. JRM: Esto es correcto (Charo).
Charito: Hicimos un reportaje sobre el particular. Ha tenido una aceptación grandísima.
Dr. JRM: A dos anos de tener este producto en la oficina nuestra y haberlo introducido en Puerto Rico y en el mundo porque fuimos con estados unidos los primeros en introducir Kybella® en nuestras practicas.
Charito: ¡Mira para allá!
Dr. JRM: Tenemos ahora una práctica muy grande de hombres que vienen a hacerse este procedimiento y hoy tenemos aquí a Guillermo Requejo.
Charito: ¿Y vamos a hacerlo aquí ahora en vivo?
Dr. JRM: Y vamos a hacerlo en vivo para que la gente sepa de que se trata.
Charito: Pero, ¿le pusiste la anestesia?
Dr. JRM: No necesita anestesia.
Charito: ¿Ya no necesita?
Dr. JRM: No necesita anestesia. La mejor anestesia es este movimiento que yo hago con los dedos . ¿Ves aquí? Que eso es pinchar y el paciente se distrae, inyecto en cada punto que ven marcado ahí, .2 cc.
Charito: En todos los puntos … y eso es grasa biliar, ¿no?
Dr. JRM: Esto es grasa, lo que se llama grasa submental, es el nombre científico de la papada, grasa que estaba debajo de la piel y encima del musculo de esa zona, que es el platisma. Aquí, la idea de tener estas líneas en colores es para el médico, si se distrae hablando con Charito (la reportera) sepa que esta en la línea negra y que sigue este orden. Todos estos puntos están separados por un centímetro. ¿Para qué? Para lograr uniformidad en el tratamiento, el radio de difusión, es decir, este producto se va a regar alrededor de un centímetro de esos puntos, cubriendo toda el área.
Charito: ¿Qué va a estar haciendo esta sustancia que se está inyectando en el área?
Dr. JRM: Bueno, lo primero que vamos a decir es que este paciente, ahora en unos minutitos va a estar experimentando un leve ardor, como un cosquilleo en esta área. Esto quiere decir que el producto esta trabajando perfectamente como queremos y va a producir una reacción inflamatoria controlada. Esa reacción permite que las células que acumulan grasa se vayan desapareciendo y el propio cuerpo las elimina. Guillermo tiene un grado de grasa submental –como ven aquí se divide entre leve, moderado a severo. Esto es un grado moderado-severo. Va a requerir posiblemente tres sesiones de inyecciones de este producto.
Charito: ¿Y cuándo estaría viéndose ya una diferencia en la reducción de la papada?
Dr. JRM: En un mes él va a notar una reducción de esto de un 30% a un 40%. Va a requerir otro tratamiento y después de ese segundo tratamiento se reduce un 20-25% y al cabo de –yo calculo que debido al grado de grasa que él tiene, en la tercera vamos a tener una reducción (Charo) posiblemente de casi el 75% a 80% de esta grasa y es permanente.
Charito: Me leíste el pensamiento. Iba a hacer esa pregunta. ¿Este efecto es permanente? ¿No necesita (qué se yo) tratamientos de mantenimiento para mantener esta área sin papada?
Dr. JRM: No. No, inclusive, personas que aumentan de peso esta área no va a crecer como crecen las otras partes (del cuerpo).
Charito: Yo estoy escondiendo la mía.
Dr. JRM: No tienes papada. Aquí lo que se hace después del tratamiento es simplemente un poco de presión, se aplica frío para evitar un poquito esa incomodidad que el paciente pueda experimentar y se le pide al paciente que en su casa utilice compresas frías. lo que va a experimentar el paciente mañana es un leve grado de hinchazón en esta área y al cabo de dos o tres semanas va a tener menos grasa submental o menos papa.
Charito: ¿Y todos los pacientes son como Guillermo? No lo hablamos con el porque está ahí quietecito para mantener el tratamiento, pero ni una lagrimita ni nada, o sea, estuvo ahí …
Dr. JRM: Bueno, yo le puedo preguntar en cámara, ¿de 1 a 10, el grado de dolor que experimentaste durante las inyecciones (Guillermo)?
Charito: ¿1, 2, 3?
Guillermo: 1.
JRM: ¡1!
Charito: ¿1? ¡Mira para allá!
JRM: Así es que es un tratamiento básicamente indoloro, rápido, la persona se puede reintegrar a sus funciones en la oficina o trabajo y algunos pacientes requieren un tratamiento, otros dos y algunos –como él tres.
Charito: Número telefónico, rápido doctor. Ah, ¿está en pantalla?
Dr. JRM: Está en pantalla.
Charito: Sí, porque eso va a ser ahora, todo el mundo va a querer estar sin papada ahora y mas para estas navidades.
Dr. JRM: Buenos, sabemos que 2/3 partes de la población (Charo) esta interesada en mejorar el área inferior de la cara o la papada.

Charito: We are ready here with Oculoplastic Surgeon, Dr. José Raúl Montes. We will see the application of Kybella® on the double chin live. He has the needle on the right hand, while holding the double chin with his left hand, to start the application This was a procedure that we introduced as soon as it arrived because you were the pioneer.
Dr. JRM: This is correct (Charo).
Charito: We did a report on the subject. It has had a great acceptance.
Dr. JRM: It has been two years since we brought Kybella® to our office, having introduced it to our practices in Puerto Rico and the United States before the rest of world.
Charito: Wow!
Dr. JRM: Now, our practice has a great number of male patients who come to do this procedure. Guillermo Requejo has joined us today.
Charito: Are you going to do it here now live?
Dr. JRM: Yes, we are going to do it live so people understand what it entails.
Charito: Did you put anesthesia?
Dr. JRM: Anesthesia is not necessary.
Charito: You do not need it anymore?
Dr. JRM: There is no need for anesthesia. The best anesthetic is this movement that I do with my fingers. See here? While I pinch, the patient gets distracted. I inject 2 cc in every point you see that has been marked.
Charito: Is that biliary fat?
Dr. JRM: This is what is called submental fat, the scientific name being jowl, which is fat that is under the skin and above the muscle in that area, which is the platysma. The idea of having these colored lines is for the doctor to ensure he follows the order in black line, in case he gets distracted talking to Charito (the reporter). All these points are separated by a centimeter. For what purpose? To achieve uniformity in treatment, the diffusion radius, i.e., this product will spread out about one centimeter from those points, covering the entire area.
Charito: What will this substance be doing to the area?
Dr. JRM: First of all, in a few minutes this patient will be experiencing a slight burning, a tingling sensation, in this area. This means that the product is working perfectly and will produce a controlled inflammatory reaction. This reaction allows the cells that accumulate fat to disappear and for the body itself to eliminate them. Guillermo has a degree of submental fat as seen here divided between mild, moderate to severe. He has a moderate-severe degree, which will possibly require three sessions of injections of this product.
Charito: When will he see a difference in the reduction of the double chin?
Dr. JRM: In a month he will notice a 30% to 40% fat reduction. He will require another treatment; after that second treatment a 20%-25% fat reduction will be seen. Due to the degree of fat he has, I calculate that after the third treatment (Charo) he will have a fat reduction of almost 75% to 80%, and it is permanent.
Charito: You read my mind. I was going to ask that question. Is this effect permanent? Do not you need maintenance treatments to keep this area without jowls?
Dr. JRM: No. No, not even people who gain weight this area. It will not grow like in other parts (of the body).
Charito: I am hiding mine.
Dr. JRM: You do not have a double chin. After the treatment, we simply put a little pressure, and apply cold pads to avoid a slight discomfort that the patient can experience; the patient is asked to use cold compresses at home. Tomorrow, the patient is going to experience a slight degree of swelling in this area; after two or three weeks he will have less submental fat.
Charito: Are all the patients like Guillermo? We have not been able to talk to him because he is staying still for the treatment. We wonder how does he feel, since we do not even see a tear.
Dr. JRM: Well, I can ask him on camera. From a scale of 1 to 10, how much pain have you experienced during the injections (Guillermo)?
Charito: 1, 2, 3?
Guillermo: 1.
JRM: 1!
Charito: 1? Wow!
Dr. JRM: So, basically it is a painless, fast treatment, the person can return to his/her daily functions at work or the office. Some patients require one treatment, others two and some like him three.
Charito: Quickly doctor, what is the phone number. Oh, is it on screen?
Dr. JRM: It is on screen.
Charito: Yes, because that is going to be the new trend, everyone will want to get rid of the double chin, even more so for Christmas.
