Ivonne: Bueno, ¿cómo puedes rejuvenecer to escote? Éste es el tema de hoy del Cirujano Oculoplastico, el Dr. José Raúl Montes, que nos acompaña en el estudio. Buenos días. Bienvenido.
Dr. JRM: Buenos días Ivonne y buenos días a todos.
Ivonne: Bueno, ¿cuán frecuente es que la edad se note en el escote?
Dr. JRM: Muy frecuente Ivonne. Básicamente en las manos y en el escote, son estas áreas sublimes donde se notan la edad de las personas y hay pocas alternativas para rejuvenecerlas o mejorarlas.
Ivonne: Pero, ¿existen tratamientos que no sean cirugía o hay cirugia para esto?
Dr. JRM: Bueno, realmente para el área del escote no existe cirugía. Los procedimientos para esta área específica son no invasivos y (tenemos) ahora contamos con una tecnología que se llama el Ulterapy. Aquí vemos unos ejemplos de esta teconología cuando anuncié su llegada a Puerto Rico, que la introducimos en mi oficina, es energía de ultrasonido para levantar la piel de la cara y el cuello. Aquí vemos un ejemplo nuestro. Ya contamos con casi 200 pacientes tratados desde que introducimos esta tecnología en Puerto Rico. Nos hemos convertido aquí en Puerto Rico en el centro principal de la región sureste de Estados Unidos y esta tecnología ha sido aprobada recientemente este año para el tratamiento del escote, lo que se conoce como el decoletage.
Ivonne: ¿Aprobada por qué organización?
Dr. JRM: Por la FDA. Es un procedimiento que se utiliza para esta área. Es calor depositado en distintos niveles en las capas finas de esta piel, de esta área donde la energía es depositada; produce calor de 70 grados para producir colágeno. ¿Qué hace el colágeno? Reestablecer las estructuras aquí, que se traducen visualmente en que las líneas pequeñas, ese grado de flacidez que se forma en esta área que es resultado obviamente de exposición solar, hábitos extrínsicos (verdad) como poca protección. Como ven aquí, éstos son los resultados que estamos obteniendo.
Ivonne: ¿En cuánto tiempo vemos cambios? Porque uno ve un tratamiento que se ve sencillo y piensa que a la semana se van a ver resultados. ¿Es rápido?
Dr. JRM: En esta área, lo interesante es que con un solo tratamiento tú puedes ver cambios como éstos que tú ves en pantalla; un tratamiento que dura de 30 a 45 minutos en la oficina, que el paciente se reintegra a sus funciones. Ustedes ven aquí, estos resultados se empiezan a observer al cabo de un mes, a los 3 meses son más obvios; pueden permanecer en esta área por el cabo de casi 2 años.
Ivonne: Precisamente, ¿este tratamiento se revierte rápidamente, permanencen estos cambios sustanciales por mucho tiempo?
Dr. JRM: Lamentablemente, todos cambiamos todos los dÍas, asÍ que cuando estamos hablando de este tipo de tratamiento, si nuestra vida fuera estática y el cuerpo (la fisiología de nosotros) parara pues se permanecía, pero seguimos cambiando. Pero es bueno establecer que este tratamiento, cuando se realiza de una manera correcta, tiene una longevidad (duración) de casi un año a dos y se puede también combinar con otras tecnologías que son importantes como es el láser para las capas más superficiales, con los peelings y ahora con la inyección de Sculptra, que es un agente bien estimulante. Así que tenemos una sinergía (combinación de tratamientos) para tratar esta área, que es tan difícil y lograr resultados tan impresionantes como éste.

Ivonne: Oculoplastic Surgeon, Dr. José Raúl Montes, joins us in the studio today to tell us how to rejuvenate your décolletage. Good morning. Welcome.
Dr. JRM: Good morning Ivonne.
Ivonne: How often is that age noticeable at the neckline?
Dr. JRM: Very frequently Ivonne. Basically, the hands and décolletage are these sublime areas where aging is noticeable; there are few alternatives to rejuvenate or improve them.
Ivonne: Is there surgery for this? Are there treatments other than surgery?
Dr. JRM: There is really no surgery for the décolletage. There are noninvasive procedures for this specific area; we now have a technology called Ultherapy. We can see some examples of this technology here, when I announced its arrival in Puerto Rico, which we introduced in my office. It is ultrasound energy that lifts the skin in the face and neck area. Here is one of our own examples. We have almost 200 patients treated since we introduced this technology in Puerto Rico. Here in Puerto Rico we have become the main center of the southeastern United States of this technology recently approved this year for the treatment of the neck area, which is known as the décolletage.
Ivonne : Approved by which organization?
Dr. JRM: By the FDA. It is ultrasound energy deposited at different tissue depths, which promotes collagen formation. This new collagen improves the appearance of chest’s skin by reducing the lines and improving the skin’s texture.
Ivonne: How soon will we you see changes? Because you see a treatment that is simple and think that within a week results will be seen. Is it fast?
Dr. JRM: Interestingly, with just a single treatment in this area you can see changes like these on screen. One treatment lasts 30-45 minutes in the office, and the patient is able to return to his/her daily functions. As you can see here, these results can be noticeable after one month, after three months they are more obvious, and they can last up to two years.
Ivonne: Does this treatment revert quickly? Do these substantial changes last long?
Dr. JRM: Unfortunately, we all change every day. When talking about this type of treatment, if our lives were sedentary and our bodies (our physiology) would stand still, then it would be permanent, but we keep changing. However, we should establish that this treatment, when done the right way, has a longevity (duration) of nearly one to two years. It can also be combined with other technologies that are important, such as laser in the top layers, peelings, and with the Sculptra injection, which is a well-stimulating agent. We have synergy (combination of treatments) when handing this area, which is hard to treat in order to achieve these impressive results.
