1ro de agosto de 2018 – Factores a Considerar Antes de Realizarse su Cirugía de Párpados


Ivonne: Bueno, hoy con nosotros el cirujano oculoplástico José Raúl Montes, quien nos habla de los factores a considerar antes de efectuarse una cirugía de párpados. Buenos días. Bienvenido.

Dr. JRM: Buenos días Ivonne y bienvenidos a todos los televidentes.

Ivonne: ¿Bueno, qué elementos debemos considerar antes de la cirugía? Me imagino que la especialidad es una de ellas y usted es cirujano oculoplástico, o sea, eso es lo suyo.

Dr. JRM: Yo creo que eso es lo más importante, Ivonne. No todos somos iguales. Tenemos todos muchas características particulares y lo vamos a ver aquí a través de muchos ejemplos. Por ejemplo, hay gente que tiene piel gruesa y esa piel gruesa también se refleja en los párpados, párpados gruesos, párpados que tienen grasa, párpados que no tienen grasa, pieles finitas en los párpados.

Como cirujano oculoplástico (es decir) aquí vemos un paciente que es el típico paciente de edad promedio que se somete a una cirugía de párpado superior con dos propósitos: función. Este paciente va a ver mejor, pero se va a ver mejor. En este paciente, que es un paciente de piel fina, se remueve solamente la piel, se deja intacto el músculo y la grasa. Lo que quiero recalar aquí (Ivonne) es que zapatero a su zapato. Hay especialidades y el que se dedica a ojos sabe cuáles son estas particularidades para que los resultados sean únicos.

Aquí vemos un paciente que se somete a cirugía de las bolsas en los párpados inferiores. Sabemos aquí que no hay exceso de piel. ¿Por qué quitar piel, por qué abrir? Se hace una cirugía interna donde no hay que quitar piel, no se abre la piel y solamente se remueve o se quita la grasa. Lo mismo en este paciente. En este paciente, esa grasa se reacomodó y se pone en áreas donde hace falta y se elimina en las áreas donde aparece como un exceso.

También tenemos pacientes con enfermedad de tiroides que pueden presentar con estas condiciones. El paciente en Puerto Rico, aunque no lo creamos, hay mucho paciente que tiene rasgos orientales en sus ojos y éste es un paciente típico. Hay que ser muy cuidadoso con el paciente de rasgo oriental porque esto debe permear, debe prevalecer en el resultado del paciente y no debemos cambiar esta etnicidad.

Ivonne: O sea, que es bastante complejo el análisis. ¿En ese caso?

Dr. JRM: Es este caso, estamos haciendo una cirugía de párpado superior e inferior porque (el paciente) en el párpado superior la cirugía tiene dos propósitos: función. La persona quiere ver mejor, quiere que el campo superior de visión esté intacto, pero también quiere verse mejor.

Ivonne: Verse mejor, claro que sí.

Dr. JRM: En esta cirugía no hay una cirugía (Ivonne) como la cirugía de párpado que presente un equilibrio mejor tan perfecto entre lo que es función y estética. Aquí (ven a) un hombre joven menos de 40 años que congénitamente tiene exceso de grasa en los párpados inferiores. En esta cirugía por dentro, sin incisiones por fuera se puede lograr eliminar esa grasa y esto generalmente es permanente.

Ivonne: El cambio es significativo. ¿La recuperación, doctor?

Dr. JRM: La recuperación en estos pacientes la mayoría de las veces es como una semana para retomar sus funciones de ir al trabajo y para hacer ejercicios, dos a tres semanas.

Ivonne: ¡Wow!

Dr. JRM: Aquí vemos por ejemplo— y yo quería presentar para que tengan una idea, de que esta cirugía yo la realizo con un instrumento muy especial. Soy el único que lo tiene aquí en Puerto Rico, que es un láser de CO2, donde a la misma vez se corta y se coagula, así que no hay sangrado. Se utiliza el láser después en un aditamento especial para recoger la piel sin tener que cortar. Aquí están viendo el procedimiento, así que es un procedimiento que es muy efectivo, que es totalmente duradero y que el paciente agradece porque va a ver mejor y se va a ver mucho mejor.

Ivonne: En el caso de estos párpados de esta joven, ¿va a permanecer esa cicatriz, queda prácticamente imperceptible, no?

Dr. JRM: Súper importante (Ivonne). Cuando tú eres un médico que conoces esa anatomía y te dedicas a esto, esa cicatriz queda totalmente escondida en lo que se llama el pliegue natural del párpado, que es donde debe estar esta cicatriz para que nunca se vea. Te voy a decir algo (Ivonne) y esto es lo más importante. El éxito de una cirugía de ojos es que la firma del cirujano tiene que desaparecer en ese canvas que es la cara de nuestros pacientes.

August 1, 2018 – Factors to Take Into Consideration Before Undergoing Eyelid Surgery

Dr. JRM: Good morning Ivonne and welcome to all the viewers.

Ivonne: What are the fundamentals to be considered before surgery? I imagine that the physician’s specialty is one of them, such as yours as an oculoplastic surgeon.

Dr. JRM: I believe that is mostly important, Ivonne. We are all different, having many particular characteristics, as we will be able to see through many examples today. For example, there are people who have thick skin, which is also reflected in the eyelids, eyelids with fat, eyelids without fat, or eyelids with thin skin.

As an oculoplastic surgeon, I may see a patient, as shown on screen, who is a typical average age patient that undergoes upper eyelid surgery for two purposes: function. This patient will have better vision, but will also have a better appearance. On this patient, who has thin skin, only the skin is removed, the muscle and fat are left intact. What I want to emphasize here (Ivonne) is to stick to what you know. There are specialties and those who are dedicated to eyes know what these particularities are in order to obtain unique results.

On this example, we see a patient who underwent lower eyelids surgery to remove those bags. Since there is no skin excess, there is no reason for skin removal or cuts? An internal surgery is done without any skin removal or opened and only the fat is removed. The same situation happened with this patient. The fat was rearranged, eliminated in the areas where it appeared in excess, and placed in areas where it was needed.

We also have patients with thyroid disease that can present with these conditions. Believe it or not, in Puerto Rico, there are many patients who have oriental features around the eyes, and this example is a typical patient. Once has to be very careful with these patients as this trait must permeate, prevail in the patient’s outcome, and the ethnicity should not be changed.

Ivonne: The analysis is quite complex. What about this example?

Dr. JRM: In this example, we did an upper and lower eyelid surgery for two purposes: function. The person wanted to keep the upper vision field intact, but also wanted to look better.

Ivonne: Look better, absolutely.

Dr. JRM: There is no surgery (Ivonne) like eyelid surgery which will provide a perfect balance among function and aesthetics. This one is a young man under 40 years old who congenitally has excess fat in the lower eyelids. Internal surgery, without incisions on the outside, was done to eliminate that fat and it is usually permanent.

Ivonne: The change is significant. Doctor, what about recovery?

Dr. JRM: Most of the time, recovery for these patients is about one week to resume regular activities, such as going to work, and two to three weeks to exercise.

Ivonne: Wow!

Dr. JRM: For example as we can see here I wanted to give you an idea how this surgery is performed, which I do with a very special instrument. I am the only one in Puerto Rico who has it, which is a CO2 laser, where it coagulates and cut at the same time, so there is no bleeding. Then, with special attachment, it collects the skin without cutting it. Here you can see that the procedure is very effective, totally durable and the patient appreciates it because he will see better and look even better.

Ivonne: What about this young woman’s eyelids? Will that scar remain? Is it practically imperceptible?

Dr. JRM: Extremely important (Ivonne). When you are a doctor who knows that anatomy and dedicate yourself to this specialty, that scar is totally hidden in what is called the natural eyelid fold, which is where this scar should be so that it is never visible. Let me tell you what is the most important element (Ivonne). The success of an eye surgery is that the surgeon’s signature has to disappear on that canvas which is our patient’s face.
