Ivonne: Bueno, si tienes los párpados superiores caídos, hoy vamos a aprender sobre la cirugía de párpados conocida médicamente como blefaroplastia. Me acompaña el Cirujano Oculoplástico, el Dr. José Raúl Montes. Un placer, bienvenido.
Dr. JRM: El placer es mío, finalmente aquí en vivo en el estudio. ¡Estoy feliz!
Ivonne: ¿Por qué esta cirugía es de las más frecuentes?
Dr. JRM: Esta cirugía es de las más frecuentes sencillamente porque la mayoría de la gente a cierta edad pierde visión por el exceso de piel o el peso del párpado sobre el eje de visión. Además, ¿por qué esta cirugía (es la más solicitada)? Mucha gente entiende que quiere verse mejor y esta cirugía es la más común que se realiza dentro de los procedimientos estéticos.
Ivonne: O sea, que hay estéticos y hay medicamente necesarios.
Dr. JRM: Función; exactamente.
Ivonne: ¿Esto es hereditario, doctor?
Dr. JRM: Buena pregunta y empezamos con el primer paciente. Esto es un paciente que congénitamente tiene un párpado que un párpado grueso, que es lo que se llama como párpado oriental, es decir, este párpado cae naturalmente sobre las pestanas. El tipo de procedimiento que se hace en este paciente es para mejorar esto, pero para preservar su etnicidad, es decir, su forma tiene que permanecer siendo la misma. En hombres y mujeres hay diferencias también. El hombre, la incisión, o dónde se hace el corte, tiene que ser más bajo que en la mujer para que el hombre conserve sus características masculinas.
Ivonne: ¿Entonces estamos hablando que cada cual tiene características faciales distintas, o sea, que en cada persona la cirugía va a ser distinta?
Dr. JRM: ¡Definitivamente! Hay que tomar en consideración la altura de las cejas. Aquí, por ejemplo, vemos una fémina después del procedimiento. Las féminas tienen las cejas más altas. Quiere decir que uno tiene que, en la cirugía, planear para que las cejas guarden concordancia con el procedimiento que uno está haciendo. Por el contrario, si las cejas están un poco bajas para empezar, hay maneras en el procedimiento para uno aumentar la altura de estas cejas.
Ivonne: Bueno, la diferencia se ve significativamente. Sabemos que varía entre paciente y paciente, pero, en términos generales, ¿de qué consiste la intervención, cortan párpado?
Dr. JRM: Esto es importante saber que esto no se sigue una receta como una receta de cocina. Cada paciente necesita algo en específico. En algunos pacientes simplemente se quita un poco de piel, se corta piel, en otros requieren remoción del músculo, otros requieren operación del músculo para subir el párpado y otros requieren remoción de grasa, pero en la mayoría de las veces conservamos la grasa. Hoy día sabemos que la grasa es importante para preservar las estructuras y la juventud del rostro,
Ivonne: Bueno antes de irnos, ¿el resultado y la recuperación, cuán rápida es?
Dr. JRM: Esto es una cirugía, primero que nada, yo puedo decir con total seguridad, indolora para el paciente el paciente se opera y se va a su casa con un post-operatorio muy cómodo. Puede hacer funciones normales que no requieran fuerza física, por lo menos las primeras dos semanas. Pueden hacer trabajos de oficina desde sus casas, así es que la recuperación es relativamente rápida. Al cabo de una semana se remueven las suturas.
Ivonne: Doctor, mucha gente se pregunta si se hace este tipo de operación si le va a quedar una cicatriz, una marca ahí en el párpado. ¿Cuál es la realidad?
Dr. JRM: Excelente pregunta, porque eso sí es una preocupación y la gente pregunta sobre queloides. Afortunadamente, en la cirugía de los parpados superiores la incisión se hace en un pliegue natural del párpado que generalmente está escondido y no se ve. Además, la piel del párpado es la más fina de todo el cuerpo. Por lo tanto, la posibilidad de desarrollar una cicatrización que sea no buena o queloides, es mínima.
Ivonne: Bueno, nos tenemos que ir, pero no me quiero ir sin preguntarle, doctor, ¿de 10 operaciones, cuántas de son peticiones de párpados?
Dr. JRM: Para mí, 100%. Éste es el procedimiento que más se realiza en hombres y mujeres en mi experiencia y en todo el mundo guarda el lugar número tres, tal vez en las cirugías más practicadas en todo el mundo. Ivonne: If your upper eyelids are drooping, today we are going to learn about an eyelid surgery medically known as blepharoplasty. Joining us today is Oculoplastic Surgeon, Dr. José Raúl Montes. It is a pleasure to have you here. Welcome!
Dr. JRM: It is my pleasure to finally be here in the studio live. I am happy!
Ivonne: Why is this surgery one of the most frequently requested?
Dr. JRM: This surgery is one of the most frequently requested, simply because most people lose vision at a certain age due to excess skin or the eyelids’ weight on the vision axis. In addition, the reason this surgery is the most requested is because many people realize that they want to look better. Thus, this surgery is the most commonly performed among aesthetic procedures.
Ivonne: In other words, there are aesthetic and medical reasons to have it done.
Dr. JRM: Exactly, mostly function.
Ivonne: Doctor, is this hereditary?
Dr. JRM: Good question. Let us start with the first patient after surgery photos. This is a patient who congenitally has a thick eyelid, commonly called an oriental eyelid, i.e., this eyelid falls naturally over the eyelashes. The type of procedure done on this patient is to improve this, while preserving the patient’s ethnicity, i.e., the form of his eyes has to remain the same. There are differences in men and women, as well. In men, the incision or cut has to be lower than in women, thereby retaining masculine characteristics.
Ivonne: We are talking about the fact that each one has different facial characteristics. That means that the surgery will be different for each person?
Dr. JRM: Definitely! The eyebrows’ height must be taken into consideration. We can see it in this example. Females have higher eyebrows. It means that as surgeons we have to plan accordingly, so that the eyebrows are consistent with the procedure that we are performing. Conversely, if the brows are a bit low to begin with, there are ways within the procedure to increase the height of these brows.
Ivonne: The difference is noticeable. We know that it varies from patient to patient, but, in general terms, what does the intervention consist of, is the eyelid cut?
Dr. JRM: It is important to note that we do not follow a recipe, like a cooking recipe. Each patient needs something specific. In some patients, only a little skin is removed, in others the skin is cut, others might require muscle removal, others might require operation of the muscle to raise the eyelid, and others require fat removal, but in most cases, we preserve the fat. Nowadays, we are aware that fat is important to preserve the face’s structure and youth.
Ivonne: Well before we go, I need to ask how quick is recovery and results seen?
Dr. JRM: First of all, I can say with complete certainty that it is absolutely painless for the patient. The patient undergoes surgery and goes home for a very comfortable post-operative period. He or she can engage in functions that do not require physical strength, at least for the first two weeks. The patient can do office work from home, so recovery is relatively quick. After a week the sutures are removed.
Ivonne: Doctor, many people wonder if this type of surgery is going to leave a scar/ mark on the eyelid. What is the reality?
Dr. JRM: Excellent question, because that is indeed a concern; people ask about keloids. Fortunately, in upper eyelid surgery, the incision is made in a natural crease of the eyelid which is usually hidden from view. In addition, the eyelid’s skin is the thinnest of the entire body. Therefore, the chance of developing keloids or poor healing is minimal.
Ivonne: Well, we have to go, but I don’t want to leave without asking you, doctor, out of 10 surgeries, how many are eyelid requests?
