Dr. JRM: Un placer para mí (Ivonne) siempre.
Ivonne: Bueno, hoy usted viene a hablar de algo que me tiene maravillada, cambios en el rostro que promete rejuvenecer, pero sin cirugía. ¡Escuchen bien esto! ¿De qué estamos hablando?
Dr. JRM: Lo más importante (Ivonne), verás que vengo acompañado. Vengo acompañado con un cráneo porque hoy no podemos hablar de este tema sin hablar de anatomía. Lo tenemos que entender es que con el paso del tiempo lo que vamos perdiendo todos nosotros sin excepción (hombres y mujeres) son los tejidos que están debajo de la piel. Piel se pone fina, grasa va desapareciendo, músculo van poniéndose fino, grasa debajo del músculo va desapareciendo y el hueso se va a reabsorbiendo. Quiere decir que nos vamos consumiendo poco a poco, paulatinamente, pero no todo es triste. Tenemos la solución para eso. Hoy día tenemos productos y hoy vamos a hablar de este producto ácido poliláctico, comercialmente conocido como Sculptra®.
Ivonne: ¿Pero, esto qué es? ¿Esto es un relleno?
Dr. JRM: Bueno, la mayoría de la gente se refiere a él (Ivonne) como un relleno, pero realmente el ácido poliláctico es mucho más que eso. Es un agente bioestimulante, como van a ver aquí. Es agua, es líquido, se mezcla con agua. Es un polvo que está en ese líquido (partículas) y uno lo va inyectando estratégicamente a distintas profundidades debajo de la piel y esto ¿qué va a hacer? Esto va a crear un armazón, va a crear estructura, va a crear un frame en la cara de las personas. ¿Qué esto hace? Va a devolverle a esta persona el volumen perdido, ya sea de grasa, de afinamiento muscular o de hueso reabsorbido.
Ivonne: ¿Pero estamos hablando de que lo devuelve de forma natural, o sea, obliga al cuerpo a producir?
Dr. JRM: Lo ideal de este producto es que el paciente no va a tener nada exógeno o extraño en su cuerpo, ni es su cara; es su propio colágeno que se estimula a cualquier edad para restablecer el volumen perdido. Aquí hemos estamos viendo algunos ejemplos de pacientes que se han sometido al tratamiento, generalmente requieren tres sesiones, separadas entre 4 y 6 semanas y al cabo de ese tiempo el paciente va a experimentar esa vuelta a la juventud y ese efecto le va a durar al paciente por un espacio de unos 3 o 4 años y después se puede retocar.
Ivonne: ¿Hay algún tipo de contraindicación en este tipo de tratamiento?
Dr. JRM: En este tratamiento no se puede inyectar en personas, por ejemplo, que tengas infecciones activas en la piel en el momento del tratamiento. También no se recomienda en pacientes que tengan condiciones autoinmunes donde requieran medicamentos donde su sistema inmune este totalmente controlado, porque parte de la idea del tratamiento es crear que ese sistema inmune cree una reacción, traer fibroblastos al cuerpo y que se cree colágeno y elastina.
Ivonne: Antes de irnos, hay un videíto a ver si lo podemos pasar rápidamente para que nos explique. ¿Esta aplicación es sencilla? ¿Se trata de inyecciones en las áreas vulnerables, en las áreas donde se entiende que hace falta el tratamiento?
Dr. JRM: Se trata de inyecciones (Ivonne) en las áreas donde el médico como un artista tiene que poder detectar dónde le va a dar al paciente lo que necesita para verse refrescado y recuerde siempre verse sobre todo natural y esto es lo que este producto permite porque es su propio tejido recreando esa juventud.

Ivonne: Joining us today is Oculoplastic Surgeon José Raúl Montes. Welcome. It is a pleasure to have you back.
Dr. JRM: It is always my pleasure to be here (Ivonne).
Ivonne: Today you will be talking about something that amazes me, i.e., facial rejuvenation without surgery. Pay attention everyone! What are we talking about?
Dr. JRM: Most importantly (Ivonne), as you can see, I brought a skull with me today because we cannot discuss this topic without talking about anatomy. It is important to understand that over time we lose, without exception (men and women), tissues that are under the skin. Skin becomes thin, fat disappears, muscle becomes thin, fat under the muscle disappears and bone is reabsorbed. It means that we are gradually decaying, but not everything is gloomy. We have the solution. Nowadays we have products and today we are talking about polylactic acid, commercially known as Sculptra®.
Ivonne: What is this? Is it a filler?
Dr. JRM: Ivonne, most people say it is a filler, but polylactic acid is really much more. It is a biostimulant agent, as can be seen in the video. It is water, it is liquid, it is power mixed with water (particles). It is injected strategically at different depths under the skin. What will it do? It will create a framework, structure, on people’s faces. What does this do? It will restore lost volume in fat, muscle refinement or reabsorbed bone.
Ivonne: Does it mean that it will return naturally, i.e., does it force the body to reproduce that loss?
Dr. JRM: The notion with this product is that the patient will not have anything exogenous or strange in his body or face. It is your own collagen that is stimulated at any age to restore lost volume. These are some examples of patients who have undergone treatment, which would generally require three sessions, separated by 4 and 6 weeks, and after that time the patient will feel their youth restored. That effect will last for about 3 or 4 years, and then they can get a touch up.
Ivonne: Is there any type of contraindication in this type of treatment?
Dr. JRM: It cannot be injected in people who have active skin infections at the time of treatment. It is also not recommended for patients who have autoimmune conditions and require medications to control their immune system, because one of the purposes of this treatment is to force the immune system to react, bring fibroblasts to the body, and create collagen and elastin.
Ivonne: Before leaving, there is a video we would like to see quickly, so you can explain. Is it a simple application? Is it injected in vulnerable areas, or areas where it is determined that treatment is needed?
Dr. JRM: Ivonne, they are injected by the doctor as an artist who can identify the areas where the patient needs to look refreshed. Moreover, it is especially important to remember to look natural. This is what this product allows because it is the body’s own tissue recreating that youth.
