Ivonne: Amigos, el Cirujano Oculplástico José Raúl Montes me acompaña hoy. Su tema: el Ultherapy®.
Dr. JRM: Buenos días Ivonne y buenos días a todos los televidentes.
Ivonne: Bueno, le llaman la nueva tecnología para el rejuvenecimiento sin invasión, sin cirugía.
Dr. JRM: Bueno Ivonne, no solamente la nueva tecnología, sino que es la tecnología aprobada por el FDA para levantamiento de la piel de la cara, cuello, cejas y algunas otras partes del cuerpo que vamos a discutir después, sin cirugía, usando energía de ultrasonido.
Ivonne: O sea, que no hay nada invasivo. Cuénteme, explíqueme cómo funciona.
Dr. JRM: Bueno, podemos empezar a ver un video para que tú veas cómo se realiza y para que ustedes tengan una idea de que este tratamiento se hace en la oficina cómodamente. Aquí van a ver a un paciente que están conociendo. Ése soy yo en mi tratamiento de Ultherapy®. Ustedes están viendo en pantalla las marcas por dónde se va a pasar este instrumento. Como ven aquí en pantalla, nosotros tenemos la posibilidad de ver aquí debajo de la piel dónde están las estructuras de tejido blando, es decir, piel, músculo, grasa y hueso. Aquí se está pasando una energía de ultrasonido que simplemente calienta debajo de la piel, estas estructuras, a una temperatura de 65° a 70° Centígrados.
Ivonne: ¿Cuál es el efecto de ese calor?
Dr. JRM: Ese calor debajo de la piel (Ivonne, muy buena pregunta) lo que produce es que el colágeno que está ya gastado, viejo, que no sirve para levantar, se renueva, se crea nuevo colágeno y ese nuevo colágeno las cadenas levantan el tejido. Es por eso que cuando el paciente se hace este tratamiento en la parte inferior de la cara y en el cuello va a observar al cabo de unas cuantas semanas (cerca de los dos a tres meses) este levantamiento. Vamos a ver aquí, por ejemplo, esta fémina, cuyo resultado es el resultado de haberse hecho el Ultherapy® en la parte inferior de la cara, el área de la mandíbula y en el cuello.
Ivonne: ¿Quiénes pueden someterse (hombres, mujeres, edades)?
Dr. JRM: Pueden someterse a este tratamiento hombres y mujeres, de edades digamos de mas de 30 años, porque edades anteriores a esa pues no hay mucha indicación, pero hemos visto porque hemos estudiado muy meticulosamente cuáles son los pacientes que son ideales. Aquí vemos otra candidata como se ve este levantamiento, esta definición en el área mandibular y en el cuello, áreas para las cuales no hay muchos tratamientos que no sean quirúrgicos o de cirugía, no invasivos.
Ivonne: Y se evita el bisturí ¿En cuanto tiempo uno tiene ese resultado?
Dr. JRM: Exactamente.
Ivonne: ¿En cuanto tiempo uno tiene ese efecto?
Dr. JRM: Esto generalmente en el tiempo que toma el colágeno nuevo en regenerarse, que se espera sea noventa días o alrededor de tres meses.
Ivonne: Bueno y en su experiencia, eso mismo le iba a preguntar, ¿es igual de efectivo en hombre como mujeres, funciona igual de rápido?
Dr. JRM: Funciona igual de efectivo en hombres y mujeres. Para darles una idea cuáles son los candidatos ideales (Ivonne), piensa en personas que tengan edades entre 40 a 60 años, que tengan buen tejido óseo, o sea, que tengan buen tejido mandibular y que no tengan una elasticidad perdida muy severa en el área inferior de la cara o el cuello. Todos esos pacientes serán buenos candidatos. Además, lo que es importante es que esto se puede combinar con otros tratamientos, como inyectables (neuromoduladores o Botox®) y también con rellenos.
Ivonne: ¿Cuánto dura esto? Una vez se regeneró ese colágeno, ¿hay que estar repitiéndolo?
Dr. JRM: Se espera que el paciente tenga una mejoría de 16 a 24 meses, es decir, 1½ a 2 años. Al cabo de este tiempo, se reevalúa y se decide si puede otra vez hacerse el tratamiento y obviamente estamos ahora explorando otras áreas del cuerpo. Esto sigue bajando. No solamente es la cara y el cuello. El resto del cuerpo es parte de todo.

Dr. JRM: Good morning Ivonne and good morning to all our viewers.
Ivonne: They call it the new non-invasive technology for rejuvenation, without surgery.
Dr. JRM: Ivonne, it is not only new technology but a technology approved by the FDA that uses ultrasound energy to lift the skin in the face, neck, and eyebrows, as well as some other parts of the body, which we are going to discuss later, without surgery.
Ivonne: So, it is completely non-invasive. Tell us how it works.
Dr. JRM: Well, we can start by watching a video, so you can see and have an idea of how this treatment is comfortably done in the office. You are seeing a familiar patient: myself during my Ultherapy® treatment. As you can see on screen the marks where this device will be applied. It allows us to see below the skin where the soft tissue structures are, i.e., skin, muscle, fat, and bone. The application of ultrasound energy is occurring, which simply delivers heat under the skin, and these structures are at a temperature of 65° to 70° Celsius.
Ivonne: What is the aftereffect of that heat?
Dr. JRM: Ivonne that is a very good question. That heat under the skin produces new collagen to replace the one that cannot be lifted because it is already worn, old; a new chain of collagen lifts the tissue. It is the reason why when a patient undergoes this treatment in the lower part of the face and neck, lifting will be noticeable within a few weeks (about two to three months). For example, in this female patient, this outcome is the result of undergoing the Ultherapy® treatment on the lower part of the face, the jaw area, and neck.
Ivonne: Who can undergo this treatment (men, women, ages)?
Dr. JRM: Men and women, 30 years and older, may undergo this treatment, because there is not much indication for anyone younger, however, we have performed studies very meticulously to determine who are the ideal patients. Here is another candidate where the definition can be noticeable in the jaw and neck areas for which there are not many non-invasive, non-surgical, treatments.
Ivonne: They can avoid the scalpel completely.
Dr. JRM: Exactly.
Ivonne: How soon before that outcome will be noticeable?
Dr. JRM: Usually, the time within which it takes for new collagen to regenerate is expected to be ninety days or about three months.
Ivonne: What about your personal experience? Is it just as effective in men as women? Does it work as fast for either?
Dr. JRM: It is equally effective for men and women. Ivonne, to give you an idea who are the ideal candidates, contemplate people who are between 40 and 60 years old, with good bone tissue, i.e., good jaw tissue, and do not have a severe loss in elasticity in the lower face or neck area. All those patients will be good candidates. In addition, most importantly, it can be combined with other treatments, such as injectables (neuromodulators or Botox®) and also fillers.
Ivonne: How long does it last? Once that collagen is regenerated, should the treatment be repeated?
Dr. JRM: The patient is expected to have improvement within 16 to 24 months, i.e., 1½ to 2 years. At that time, after a reevaluation, it is decided whether another treatment is recommended. Obviously, we are now exploring other areas of the body, not just the face and neck. We are expanding our research to the rest of the body.
