Ivonne: En las últimas semanas, ha sido noticias, el posible efecto secundario que pudiera tener la vacuna del Covid-19 en pacientes con rellenos faciales, pero para ofrecernos los detalles nos acompaña vía Skype el doctor José Raúl Montes, que es cirujano oculoplástico. Buenos días. Bienvenido.
Dr. JRM: Buenos días Ivonne y bienvenidos a todos ustedes. Gracias por estar aquí.
Ivonne: La FDA advierte que personas con rellenos faciales cosméticos pudieran sufrir de reacciones adversas, según ha demostrado el estudio clínico de la vacuna Moderna. Primero, ¿se sabe qué tipo de relleno o cualquier relleno facial?
Dr. JRM: Realmente, cualquier relleno facial puede ser el agente incitador de una reacción. Ahora, me gustaría (Ivonne) que pasáramos a unas gráficas para que veas cuáles son este tipo de reacciones, que son principalmente —la foto número 1, ustedes van a ver que todas las noticias, cuando se hacen estudios, lo que resaltan son los side effects. Aquí ven en las noticias ya dónde Moderna está estableciendo que hay reacciones adversas a esta vacuna. Veamos cuáles son las reacciones adversas en pacientes que se han colocado sustancias de rellenos.
Básicamente, las reacciones que se han visto son hinchazón en la cara o lo que se conoce como angioedema, que es hinchazón excesiva en el área de los labios —y sabemos que los labios es una de las áreas que más se inyecta con rellenos faciales. Veamos otra foto para que vean unos labios y esta foto es más gráfica. Esto es una dermatóloga que amablemente otro dermatólogo colega de los Estados Unidos tuvo este caso de esta dermatóloga y me facilitó la foto. Para que ustedes vean cómo es la reacción de un paciente que ha tenido previamente inyección de sustancias de relleno en los labios y lo que puede ocurrir a un día o dos días después de la inyección.
Ahora, veamos los números, que es lo importante aquí. Si nosotros vemos lo que pasó en los trials, que son los estudios para aprobar esta vacuna, Pfizer, por ejemplo, probó a 46,000 sujetos de varios países. No hubo reacción ninguna asociada a rellenos faciales. La de Moderna tuvo 15,000 participantes, de los cuales solamente tres tuvieron este tipo de reacción. Fueron tres féminas de edades de 51, 46 y 29 años y sólo una de ellas tuvo hinchazón facial; las otras lo que tuvieron fue hinchazón en los labios.
Ivonne: Okay, teniendo es información doctor, ¿qué se hace? ¿Se da tratamiento, se debe ingerir algún preventivo, las personas que tengan algún tipo de inyectable antes de vacunarse?
Dr. JRM: Esa es la pregunta clave aquí y lo que le quiero llevar de mensaje a todos ustedes. Miren este número, 0.019%. Eso es un número bien bajo y eso lo que quiere decir es que ambas vacunas tienen un perfil de seguridad altísimo y son efectivas. Estas ocurrencias son bien raras, son pocas y en la mayoría de los casos —o yo diría en todos los casos (aquí ven los tres casos: 51, 46 y 29 años)— volviendo a tu pregunta (Ivonne) los tres casos resolvieron en 24 a 48.72 horas con el uso de antiestamínicos. Usted, después de la vacuna si usted ve una reacción rara, que se hincha la cara, se hinchan los labios, usted toma un antiestamínico (Benadryl). Hay muchos de estos que son OTC (over-the-counter), no son recetados. Después de tres o cuatro días, si la hinchazón no resuelve, su médico puede darle un medicamento recetado como cortisona, pero ‘ojo’: la vacuna es importante, hay que vacunarse. Estas reacciones son raras y se resuelven. Ahora, es importante que su medico sepa reconocerlas y sepa tratarlas.
Ivonne: Doctor, ¿ha tenido usted alguna experiencia aquí en Puerto Rico de alguien vacunado con algún tipo de reacción al día de hoy?
Dr. JRM: Precisamente, ayer recibí una llamada de una paciente que hace nueve meses se había inyectado fillers en los labios y hace dos días se puso su vacuna y en 24 horas desarrolló angioedema, o sea, la hinchazón en los labios. A esta paciente, obviamente, le recomendé el uso de antiestamínico y parece que se resolvió el problema.
Ivonne: Bueno, doctor, muchísimas gracias por estar disponible y sobre todo por orientarnos a todos en torno a este tema.
Dr. JRM: Ivonne, si me lo permites, lo que quiero es agradecer a todos los centros de vacunación, a todos los hospitales, colegas, a toda la gente envuelta para este esfuerzo masivo. Yo espero que todo Puerto Rico —estamos de los primeros lugares en el mundo logrando vacunar a nuestra gente en por ciento poblacional y de una manera excelente— así que los felicito a todos, a mis colegas, vacúnense todos, por favor. Es nuestra medida de combatir esto y Pavía Santurce ha hecho una labor extraordinaria y los quiero reconocer. Gracias a todos.

Ivonne: We have seen it on the news in the last few weeks, the possibility of a side effect that the Covid-19 vaccine could have in patients with facial fillers. To discuss the details, Dr. José Raúl Montes, who is an oculoplastic surgeon, joins us via Skype. Good Morning. Welcome.
Dr. JRM: Good morning Ivonne and welcome everyone. Thanks for being here.
Ivonne: The FDA issued a warning for people who have had cosmetic facial fillers that they could suffer from an adverse reaction, as Moderna’s vaccine clinical study has shown. Firstly, do you know if it is for a particular filler or any facial fillers?
Dr. JRM: Actually, any facial filler can trigger a reaction. Ivonne, I would like us to turn to some graphs, so that you can see what these types of reactions. Mainly, as seen in the first photo, you will notice that when discussing studies, the news emphasizes on the side effects. As you can see here, Moderna has already established that there are adverse reactions to this vaccine. Let’s see what are those adverse reactions in patients who have had fillers.
Basically, the reactions that have been seen are facial swelling or what is known as angioedema, which is excessive swelling in the lips -—and we know that the lips are one of the most injected areas with fillers. Let’s look at another photo, so that you can see another example; this photo is more graphic. A colleague dermatologist from the United States kindly shared this photo of a case with an adverse reaction. You can see how this patient reacted after having previously had filler substances injected into the lips, and what can happen a day or two after the injection.
Now let’s look at the numbers, which is of upmost importance. What happened in the trials, which are the studies to approve this vaccine, Pfizer, for example, tested 46,000 subjects from various countries. There was no reaction associated with facial fillers. Moderna had 15,000 participants, of whom only three had this type of reaction. There were three females 51, 46 and 29 years old, and only one of them had facial swelling; the others had swelling of the lips.
Ivonne: Having this information doctor, what do you do? What is the treatment given? Should people who have had some type of injectable before getting vaccinated take a preventive medication?
Dr. JRM: That question is key question and is what I want to address to everyone. Look at this number, 0.019%. That is a very low number; what it means is that both vaccines have a very high safety profile and are effective. These occurrences are very rare. There are a few cases (here you see the three cases: 51, 46 and 29 years). Ivonne, in most if not all of the cases, like these three, it resolved in 24 to 48.72 hours with the use of antihistamines. After the vaccination, anyone that sees a rare reaction, which is swelling of the face and/or lips, should take antihistamine (Benadryl). Many of these that are OTC (over-the-counter), do not require a prescription. After three or four days, if the swelling does not resolve, your doctor can give you a prescription for a medication, such as cortisone. However, be on the look-out, getting the vaccine is important, get vaccinated. These reactions are rare and resolve themselves. Moreover, it is important that your doctor know how to recognize them and treat them.
Ivonne: Doctor, have you had anyone experience it here in Puerto Rico, someone who got vaccinated and had some type of reaction?
Dr. JRM: Just yesterday I received a call from a patient who had injected fillers into her lips nine months ago, and two days ago she got vaccinated. Within 24 hours she developed angioedema, i.e., swelling on the lips. Obviously, I recommended the use of antihistamine. It seems that the problem has been resolved.
Ivonne: Well, doctor, thank you very much for being available and, above all, for guiding us regarding this topic.
Dr. JRM: Ivonne, if I may, I want to thank all the vaccination centers, hospitals, colleagues, and all the people involved in this massive effort. I hope that everyone in Puerto Rico can get vaccinated. We are among the first in the world, in terms of percentage of population, managing to vaccinate our people, and in an excellent way. Thus, I congratulate all of my colleagues, and urge everyone to get vaccinated, please. It is our way to fight this, and I want to acknowledge Pavía Santurce, which has done an extraordinary job.
