Ivonne: Amigos, un tema interetasantísimo. Con los años, los labios van perdiendo forma y volumen. El Cirujano Oculoplástico José Raúl Montes nos habla de un método natural que promete cambios notables. Buenos días. Bienvenido.
Dr. JRM: Buenos días Ivonne. Buenos días a todos los televidentes.
Ivonne: Bueno, ¿por qué los cambios en los labios con el pasar de los años?
Dr. JRM: Bueno, yo siempre digo que en la cara (Ivonne) siempre tenemos de foco de atención los ojos y con los ojos las cejas, pero en la parte inferior de la cara son los labios los que denotan la edad y el tono de cansancio o dencanso de un paciente. ¿Por qué los labios cambian con el tiempo? Sencillo, es un área de mucho movimiento, el músculo se afina, las paredes superficiales, es decir, la piel se afina también. Tenemos también otra cosa. Aquí vemos, por ejemplo, en esta gráfica la década de los 20 años, 30, 40, 50. Si vemos, cuando el paciente es más joven, los labios se ven carnosos, lo que se llama más protuberantes, mas obvios. La proyección de la parte superior que parece un arco de cupido (se llama así mismo el arco de cupido) es pronunciada igual que las columnas arriba. Con el paso del tiempo se va perdiendo volumen, se van metiendo hacia adentro. Ese arco se va poniendo más llano y se va perdiendo esa proyección y las esquinas de la boca van perdiendo soporte. La persona se empieza a ver, no solamente de más edad, se empieza a ver cansada.
Ivonne: Bueno, ¿qué método usted promueve? Se trata de un método, me dicen que es natural y que cambia la apariencia.
Dr. JRM: Aquí lo estamos viendo en pantalla. Es natural porque stamos hablando del producto aprobado por la FDA que se conoce como Restylane® Silk. Silk como la palabra seda, que es un producto constituido y hecho precisamente para esta área fina donde las inyecciones se tienen que hacer muy superficiales para que se vean uniformes y se vean naturales. Aquí estamos hablando de reestablecer el volumen de los labios, en paciente joven, en pacente de mediana edad y en paciente mayor donde los labios se han metido o escondido hacia adentro.
Ivonne: ¿Esto no requiere cirugía y el cambio es inmediato?
Dr. JRM: El cambio es inmediato. Aquí vemos en pantalla la paciente inmediatamente después del procedimiento en la oficina y el cambio que le da a estos pacientes es bien positivo porque el paciente se siente rejuvenecido al momento. Los labios comunican mucho en la persona, tienen la capacidad de decir, por ejemplo, si la persona está saludable, si tiene una exposición al sol controlada y también los fumadores. Los fumadores presentan cambios en esta área que son mucho mayores y acelerados que en el resto de la población.
Ivonne: Me queda poquito tiempo, pero estos labios que lucen poco naturales, exageradamente protuberantes, ¿son errores en el proceso y es este tipo de proceso o son otros métodos?
Dr. JRM: Esto ocurre, desgradaciamente, por dos razones principales: 1) Productos que no son adecuados para esta área, por ejemplo, nunca se inyectan productos que son permanentes en esta área. Realmente, como regla general en la cirugía cosmética, no se inyecta en la cara productos permanentes porque seguimos cambiando con el tiempo. 2) Inyecciones que no son hechas por personas que están debidamente entrenadas para estos procedimientos que requieren mucha delicadeza en su manera de ejecutarlo.

Ivonne: Friends, we have an interesting topic today. Over the years, your lips loose their shape and volume. Oculoplastic Surgeon José Raúl Montes is with us today to talk about a natural method that promises remarkable changes. Good Morning. Welcome.
Dr. JRM: Good morning Ivonne and good morning to all viewers.
Ivonne: Why are there changes in the lips over the years?
Dr. JRM: I always say that our face is the focus of attention, especially in the eyes and eyebrows. However, in the lower part of the face, the lips is the area that shows the aging and restlessness of a patient. Why the lips? Simple, it is an area with a lot of movement, muscle thinness and the skin loses its thickness. On this example, showing a person in their 20s 30s 40s and 50s, you can see that when the patient is younger, the lips are fuller, more protruding, more obvious. What is known as Cupid’s bow is protruded, and through time it looses volume, poking inward. The bow gets flatter and loses projection. The mouth’s corners lose support. The person looks tired and begins to show the signs of aging.
Ivonne: What method do you promote? I hear that it is natural and changes our appearance.
Dr. JRM: We can see it on screen. It is natural because it is FDA-approved, known as Restylane® Silk. It is silk because it is created precisely for this fine area where injections have to be used to create a natural look. We are talking about restoring volume to the lips in young, middle-aged and elderly patients, where the lips have hidden inside.
Ivonne: Does this require surgery? Is the change immediate?
Dr. JRM: The change is immediate. We can see on screen this patient after a procedure in the office and the positive changes in her. She feels rejuvenated. Lips communicate a lot and they have the ability to say, for example, if the person is healthy, the amount of son exposure she has had, and whether she is a smoker. People who smoke will show more accelerated changes in this area than the rest of the population.
Ivonne: I am running out of time, but what can you tell me about lips that are protruding and do not look natural? Are these errors in the process and is this that type of process or are those other methods?
Dr. JRM: Unfortunately, this occurs for two main reasons: 1) Products to be injected in this area cannot be permanent. Actually, as a general rule in cosmetic surgery, nothing permanent should be injected to the face, because people change over time. 2) Only people who are properly trained should inject these products because they must be gentle in the execution.
